I acknowledge that fitness activities and using of the equipment and machinery with knowledge of the dangers involved. I hereby expressly assume any and all risks of inhere or death. I hereby do further declare myself to physically sound and suffering from no condition, impairment, disease, infirmity or other illness that would prevent my participation or use of the equipment to machinery. I do hereby acknowledge that I have been informed of the need for a physicians approval for participation in an exercise/fitness training activities or in the use of exercise equipment or machinery. I also acknowledge that it has been recommended that I have a yearly or more frequent physical examination and consultation with a physician as to physical activity, exercise and use of exercise activities and equipment use.
I acknowledge that I have either had a physical examination and been given a physicians permission to participate, or that I have decided to participate in activities and use of equipment and machinery without the approval of a physician and do hereby assume all responsibility for participation, activities, training sessions, bootcamps and utilization of equipment and machinery in my activities to include any workout, group session, bootcamps, and/or personal training activities conducted by Sarah Young NO MATTER THE LOCATION. In consideration with my use of the personal training coach I agree to forever release and discharge Sarah Young, Forever Young Fitness, it’s officers or agents (to include but not limited to…Elgin School District, Pride Fitness, Fierce Fitness) stated above from any and all claims, suits, or potential suits liability, damages, or injury to my person or property resulting in such use, or personal advice from Sarah Young, other employees, or owners whether known or unknown. By signing this waiver, I release and forever discharge Sarah Young, Forever Young Fitness, it’s officers, agents, employees, representatives, executors and all other from injuries or damages arising out of or connected with my participation in all activities, use of equipment or machinery, training sessions or any act or omission, including negligence by representatives.
All payments are NON REFUNDABLE
For 1:1 clients: You do have from time of payment until the first session to receive 100% of payment refunded, but once sessions have begun payment is no longer refundable.
For Small Group workouts: NON-REFUNDABLE
